Adding an activity to a licence
Consumer Affairs and Fair Trading MUST be notified of your new address within 30 days of moving. You need to complete an “Application to amend registered details” form.

You must provide a certified copy:
change of name certificate issued by the Department of Births, Deaths and Marriages
a marriage certificate
for companies a certificate issued by ASIC showing the name change
Consumer Affairs and Fair Trading MUST be notified when you change your employer details. You need to complete an “Application to amend registered details” form.
To have the condition of provisional removed you need to have completed all the units of competency for each activity listed on your licence.
Once you have done the training you complete the form “Application to amend registered details” form and attach a copy of your statement of attainment issued by your training provider.
The condition of licensed premises only can only be removed by adding the details of a crowd control agent. This is done by completing an “Application to amend registered details” form.
If you hold a current licence you may request to have an additional activities added, once it is endorsed on your licence this will allow you to then undertake those activities.
To add an activity you need to complete the application to amend details form.
The licence period for new activities will be back dated to the commencement date of your current licence and will expire on the same date.
IMPORTANT. If there is a prerequisite training requirement for a licensed activity you must meet this prerequisite to have a licensed activity added. You must therefore provide a statement of attainment that meets the prerequisite standards with this application.
